MEDICARE WELLNESS VISITS / UPDATED SEPTEMBER 2026
Annual Wellness Visit checklist
An AWV develops or updates a personalized prevention plan. It is not a routine physical and does not automatically make every screening or assessment separately billable.
1. Verify the correct service
- G0402: the Welcome to Medicare IPPE during the first 12 months of Part B.
- G0438: the initial AWV, subject to enrollment and prior-service eligibility.
- G0439: a subsequent AWV. Verify the payer's next eligible date and prior claims, including those from other practices.
2. Complete the required prevention work
Use the CMS checklist for the selected visit: health risk assessment, relevant history and measurements, cognitive assessment, risk review, and a personalized screening/prevention plan. Initial and subsequent requirements differ. This summary does not replace the full CMS component list.
3. Review additional services individually
A separate problem-oriented E/M needs significant, separately identifiable work. Apply modifier 25 to that E/M when appropriate. Check each other service's eligibility, documentation, bundling, frequency, and cost-sharing rules. Do not automatically add an HRA or screening code for work already included in the AWV.
