When to use it
Same-day admit-and-discharge encounters of low complexity:

brief observation for chest pain rule-out with negative workup, brief admit for IV hydration with rapid resolution, post-procedure observation with uneventful course. The patient must be admitted and discharged on the same calendar day, regardless of whether status was inpatient or observation. The 2023 unified rules consolidated the prior 99218-99220 and 99234-99236 splits.
Documentation checklist
- ✓MDM low complexity OR 45 minutes total time on the date of the encounter.
- ✓Document both the admission decision and the discharge decision, including discharge instructions and follow-up plan.
- ✓Same-date admission and discharge is mandatory; if the patient stays overnight, use 99221 + 99238/99239 separately.
Common pitfalls
- !Billing 99221 + 99238 on the same calendar day instead of 99234-99236. Same-day admit/discharge requires the combined code.
- !Up-coding to 99235 or 99236 without supporting MDM elements.
Payer notes
Medicare requires the patient be admitted and discharged on the same calendar day. The 8-hour minimum rule (formerly required for observation status to qualify for same-day admit/discharge codes) was clarified in the 2023 unified guidance; verify documentation of duration of stay.
